This project provides students an opportunity to identify an aggregate within a certain geographic area and to conduct an assessment, identify health problems and plan appropriate interventions. The community assessment, analysis and care plan project is a team project.


The community assessment, analysis and care plan project is a team project. Each clinical group will divide into two groups, each selecting an aggregate to be studied. The selection must be approved by the clinical professor.

Procedure

1. Community Assessment and Analysis. Identify an aggregate and determine how information pertaining to identification of health problems was collected. Discuss the following:

• Information needed to determine appropriate interventions
• How information was collected (use of Windshield Survey and other resources)
• Timetable used for collecting information

2. Parameters. Identify geographic area studied. While conducting an assessment, compile and analyze factors which include, but are not limited to, the following:

• Results of Windshield Survey
• Population characteristics
• Environment, housing, industry, noise
• Religious institutions
• Transportation
• Health care resources

3. Analyze the community's health statistics and compare them to the health statistics of the county, state and nation.

4. Clearly identify the strengths and weaknesses of the community being studied.

5. Community Care Plan

• After a complete data analysis and comparison, clearly identify the health-related problems within the community, giving rationale for each.

• Propose population-based interventions for no more than 3 identified problems. Address all levels of prevention when planning interventions.

• State expected outcomes.

• Compose a plan for implementing the interventions.

• Prepare an evaluation plan.

6. Structure

• Organization of composition: Include a Table of Contents. Incorporate headings for sections. Use APA guidelines for headings, table, or charts. Use Appendices for detail and additional information. Cite sources and references using APA format. Include Reference List. Maximum length: 20 pages.

7. Peer Evaluation. Each student should use the attached form and turn in to the instructor in a separate sealed envelope.
